POSITION TITLE: Resident Assistant

SUMMARY: Provide service, assistance and support to the residents.  Coordinate and schedule work and supervise delivery of care service.  Order housekeeping supplies.  Administer routine medications to residents in accordance with state regulations.

SUPERVISOR: Program Manager

ESSENTIAL JOB RESPONSIBILITIES         

  1. Follow the service plan in providing services to residents.
  2. Assist in the development and updating of assistance plans.
  3. Ensure that service plans are completed and updated in a timely manner.
  4. Coordinate and adjust housekeeping and laundry schedules as needed.  Assign resident-services tasks.
  5. Do personal laundry for residents if so requested on the service plan.
  6. Strip and make residents’ beds weekly; launder residents’ linen and towels, fold and return them to resident.
  7.  Give residents prescribed medication or remind them to take medication.
  8. Assist in preparing meals, serving meals and bussing tables during mealtimes.
  9. Clean residents’ apartments on assigned days and as outlined in the service plan
  10. Encourage socialization among residents
  11. Assist with the activity program; conduct activities as directed by the Activity Coordinator or assigned by supervisor.
  12. Encourage and/or remind residents to participate in activities.  Promote activities enthusiastically to pique residents’ interest.
  13. Chaperone activities outside the facility when requested.
  14. Monitor health, safety and well-being of residents.
  15. Document information pertaining to changes in residents’ functional status, personal needs, appointments, etc.
  16. Assist in keeping the environment safe for residents.
  17. Provide one-on-one support/visitation to assigned residents on a weekly basis or as requested by supervisor or as specified in the service plan or daily communication log.
  18. Report concerns about residents to supervisor and coworkers.
  19. Respond to resident emergencies; provide first-aid assistance and arrange for appropriate medical attention/follow-up.
  20. Identify and order supplies needed within annual budget.  (Prior approval from supervisor is needed for purchases that are in excess of the budget).
  21. Maintain an accurate record of chargeable services (cleaning, personal laundry, guest meals, guest room use, etc.).  Submit record to supervisor by the designated date each month.
  22. Assist in the development of staff work schedules.
  23. Consults with RN on residents health care issues.
  24. Practice safety in working around and with equipment, chemicals, tools and utensils.
  25. Carry out other duties as assigned by supervisor.

Job Information

Position Title: Activity Aide

Summary: Assist Life Enrichment Coordinator with resident activities to provide life-enriching events for residents and staff.

Supervisor: Community Development Coordinator (CDC)

Essential Job Responsibilities

  1. Assist CDC with planning and implementing activities in the following categories: physical (exercise), social, recreational, intellectual/educational, creative, entertainment, religious, service/volunteering and special requests of the residents
  2. Facilitate socialization among residents.
  3. Invite and remind residents to participate in planned activities.
  4. Promote activities in a way that piques residents’ interest.
  5. Conduct activities outside the facility.
  6. Conduct/lead group activities.
  7. Plan & organize ongoing & new activities.
  8. Assist CDC with the implementation and decoration of the facility for special events, seasons and holidays.
  9. Transport residents for 1:1 activity, outings, regularly scheduled group trips and doctors appointments.
  10. Carry out other duties as assigned by supervisor to ensure smooth operation of the program and to meet residents’ needs.
  11. Chart all 1:1 activities and group logs & ensure they get turned into the CDC in a timely manner.
  12. Provide appropriate beverage & food options at meal times, snacks & on outings.
  13. Distribute cigarettes appropriately and on time.

    Qualifications

    1. Experience in activity planning with older adults preferred.
    2. Experience and interest in working with older adults.
    3. Experience in volunteering.
    4. Evidence of creativity.
    5. Demonstrated organizational skills.
    6. Mature, pleasant, friendly and cheerful personality.
    7. Proof of car insurance. It is recommended that you carry at least $300,000.00 worth of liability insurance.

Title Licensed practical nurse (LPN)

Abundant Life Manor is looking for a independent, dependable & responsible person to provide personal care and support to our residents. Under the general supervision of the Registered Nurse, the LPN is responsible for delivery of routine nursing care to the residents of Abundant Life Manor. The duties are performed in conjunction with the care team and as outlined in the plan of care. Abundant Life Manor is a 42 bed Behavioral Health CBRF located on Milwaukee’s East side.

This is for a 2nd, 3rd or split shift position. We can be VERY flexible & creative with hours.

We have a full & part time position available; must be able to work every other weekend and some holidays.

Responsibilities

  • Routine direct nursing care – providing for the emotional and physical comfort of the resident including bathing, incontinent care, assistance with IADLs/ADLs and housekeeping.
  • Observing, reporting and documenting to the RN a resident’s change of condition, including physical and behavioral changes.
  • Monitor medication refusals and notify appropriate personnel.
  • Assist in the supervision of direct care staff to assure that the residents direct care needs are addressed.
  • Medication Administration and monitoring – to assure that policies and procedures are being followed and the Medication Aides are properly trained.
  • Following the resident plan of care.
  • Work in conjunction with the care team.
  • Supervise and maintain a safe environment for residents and staff.

Qualifications

  • Graduated from an accredited school of nursing.
  • Licensed to practice in the State of Wisconsin.
  • Experience with behavioral health is preferred.
